Forgetting Patient's Name

Addressing the patient with his/her last name is very important. If you some how forget the patients name what you can do is either

Look for the patient's information chart in the examination room and read the name form there. This is not a very good option, the patient may notice you reading it again, you may get confused failing to find it, or it can cause you to waist precious time.

In real life it some times difficult for the residents to remember the names of all the patients; on the floor for example. So they address the patients by calling them Sir or Madam. So instead of getting confused,using this option is also not bad.

TIP: Before entering the room and starting the encounter, juts write the patient's last name in capital letters and encircle it on top of the paper provided to you by the test center authority.
In this way you can read the name if you think you have forgotten it.
